These forms are an easy way for Dr. Maria to assess your current well being. You can print them out at no charge and take some time to review the questions. We encourage everyone interested to fax them to our office.  We will gladly call you to book FREE 15 minute phone consultation about your assessement.

MAF - Metabolic Assessment Form.  This allows us to look at symptoms you may be experiencing that may be caused by organ or hormone imbalance. Adrenals, blood sugar, stomach, thyroid, pituitary, prostate, and others are common things we work with.

NTAF - Neurotransmitter Assessment Form. This form allows us to begin to pinpoint what areas of the brain are being affected as well as which neurotransmitters may be needing some nutritional support as they affect your moods and quality of life. Again, this can all tie back to the system as a whole.  That is why we like patients to fill both forms out.

CNNQ - Child Neurotransmitter & Nutrition Questionnaire.  Behavioral, mood and immune system concerns are easily addressed when we know the imbalances.
